Overview

Pharmacist

Willamette Valley Cancer Institute is looking for a Pharmacist to join their team. As the center grows and expands - offering more advanced treatment therapies and participating in cutting edge clinical trials - a strong pharmacist is needed to keep up with the increased clinical support needed for patient care. Our team of pharmacists provide care in our medically integrated pharmacy as well as play a crucial role in our patient care by clinically advising on our pharmaceutical regimens. A strong candidate has significant experience in a specialty retail setting, as a clinical pharmacist, in IV admixture, or a combination of those. While oncology experience is strongly preferred, experience in a complex medical specialty can be transferable.

The general pay scale for this position at WVCI is $105,820-$174,138. The actual hiring rate is dependent on many factors, including but not limited to: prior work experience, education, job/position responsibilities, location, work performance, etc.

Responsibilities

  • Performs initial patient evaluations and work-up as it pertains to disease status and anticipated prescribed therapy.
  • Collects, analyzes, interprets, and evaluates appropriate data relative to patient therapy. Obtains histories and other patient data to optimize drug therapy.
  • Establishes therapeutic objectives and monitors patient for desired therapeutic outcomes. Evaluates the significance of signs and symptoms relative to disease processes and recommends modifications in drug therapy.
  • Detects and evaluates the clinical importance of adverse reactions and complications associated with drug therapy. Provides counseling and answers questions from patients, family and health care professionals concerning drug therapy.
  • Maintains active involvement with clinicians in the development and implementation of a standard drug order format and ordering process.
  • Maintains thorough and appropriate documentation in the patient clinical chart of physician orders and communication with other health care professionals.
  • Coordinates with physician staff and service providers in planning and implementing patient care plans to ensure safe and timely delivery of drug therapy.
  • Reviews and performs QA checks on all new and repeat medication orders. May prepare or supervise the preparation and dispensing of drug therapy according to company policies and procedures.
  • Participate in anticoagulation monitoring
  • Administer vaccines as needed
  • Participate in prescription procuring and dispensing programs including DUR
  • Prepare and review educational materials for staff and patients
  • Presents training and in-service programs as necessary.
  • May supervise the activity of pharmacy or admixture technicians.


Qualifications

  • Bachelors or Pharm. D. degree from an accredited school of pharmacy.
  • Minimum 2 years of experience as registered pharmacist.
  • Hospital or clean room experience preferred.
  • Retail experience preferred.
  • Experience in a pharmacy with I.V. skills, oncology, or specialty area background strongly preferred.
  • Valid state licensure as a registered pharmacist.
